Any paperwork not received may result in your child(ren) being unable to participate upon the commencement of the season. The deadline for all paperwork to be completed and returned is June 23.  

Participant Form & Medical Form
Please schedule your physicals early! The Medical Release & Treatment form must be dated after January 1, 2008. The physician's name, address, and telephone number must be stamped or printed and signed by your physician. While the actual physical may be last year's physical (dated after January 1, 2007), physician permission to participate dated after January 1, 2008, is required.

NOTE: Substitutions to this Medical Release & Treatment Form will NOT BE ACCEPTED!

For anyone interested in a Board position, Coaching position, or Team Parent position. Must be complete and accompanied by a copy of your driver's license.
